Document the skilled interventions delivered this visit; each should connect to a finding and carry a reassessment in Section 10.

  • Comprehensive perfusion and neurovascular assessment with side-to-side comparison and trend interpretation.
  • Doppler pulse assessment when a pulse is not palpable.
  • Compression application or adjustment per order, with skin checks beneath the garment.
  • Positioning per order — elevation for venous edema, and a neutral or dependent position only when specifically ordered for arterial insufficiency.
  • Wound assessment and care coordination for arterial or venous ulcers per order.
  • Medication reconciliation, including a bleeding-risk and interaction screen for anticoagulants and antiplatelets.
  • Coordination of vascular studies or laboratory monitoring per order (ankle-brachial index, duplex ultrasound, INR).
  • Fall-risk and activity guidance for claudication, including a structured walking program where ordered.
