- Appetite and intake — amount, recent change, early satiety, and food preferences and aversions.
- Therapeutic-diet understanding and adherence, and any difficulty following it.
- Unintentional weight change — amount and timeframe (corroborated with the measured trend).
- Barriers — food access and finances, the ability to shop and prepare meals, and dentition.
- GI symptoms — nausea, vomiting, early satiety, or constipation affecting intake.
- Enteral feeding — tolerance, the schedule, and any difficulty with the regimen.
