10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

• The effect of an intervention — for example improved intake, supplement tolerance, or weight stabilization on the trend.
• Tolerance of an enteral regimen change or a supplement.
• Teaching effectiveness: teach-back percentage or return demonstration of the diet or enteral technique.
• Functional or symptom response — reduced early satiety or nausea, or improved energy.
• The plan for reassessment timing and what will be re-evaluated next visit.
