Add these anticoagulation-specific elements at each visit alongside the parent examinations — this is not a repeat of the full exam:
Bleeding surveillance: skin (bruising, petechiae), gums, epistaxis, hematuria, melena or hematochezia, and hemoptysis; a fall and head-injury screen.
Thromboembolic surveillance per indication: deep-vein thrombosis signs (Peripheral Vascular), stroke or embolism symptoms (Neurological), and atrial-fibrillation rate and rhythm (Cardiovascular).
INR result reviewed and trended against the ordered range (warfarin); renal function reviewed against the ordered schedule (DOAC).
Medication adherence; a new-medication and OTC or supplement interaction screen (NSAIDs, certain antibiotics and antifungals, amiodarone, and vitamin-K-containing foods for warfarin).
Dietary vitamin-K consistency (warfarin) and an alcohol screen.
