4. Medication and treatment monitoring

Monitor the disease-specific therapy for effect and adverse effects; report values outside the ordered parameters. Exact monitoring intervals follow the provider's orders.

  • Warfarin: INR per the ordered schedule and range; dose adjustments per the provider; vitamin-K dietary consistency; and extensive drug and food interactions.
  • DOACs (apixaban, rivaroxaban, dabigatran, edoxaban): renal function (creatinine clearance) per the ordered schedule; no routine INR; adherence is critical because of the short half-life; fewer dietary interactions but important drug interactions remain.
  • Bleeding management: hold or reversal per the provider's orders; report any supratherapeutic INR and any bleeding.
