5. Skilled teaching priorities

Document each teaching point with topic, method, audience, and measured mastery (teach-back percentage or return demonstration), per Foundations.

  • Bleeding precautions — soft toothbrush, electric razor, fall prevention, and avoiding contact injury — and the warning signs of major bleeding.
  • Adherence (especially DOAC timing) and not stopping the anticoagulant without provider direction because of thrombosis risk.
  • Interaction avoidance (NSAIDs, certain antibiotics and antifungals, and supplements) and vitamin-K dietary consistency for warfarin.
  • The INR-monitoring schedule and follow-up, and when to seek emergency care (major bleeding or a head injury).
