Subjective — ask

  • Bowel pattern — the last bowel movement, frequency, consistency, and any change; constipation or diarrhea.
  • Nausea and vomiting — frequency and character, including any blood or coffee-ground appearance.
  • Appetite and early satiety (detailed intake in Nutrition), heartburn, and difficulty swallowing.
  • Abdominal pain (characterized in Pain) and any blood in the stool or black stools.
  • Ostomy function — output, appliance fit, and any leakage or skin irritation.
