Document each teaching point with topic, method, audience, and measured mastery (teach-back percentage or return-demonstration result), per Foundations — Teach-back and return demonstration.

  • The bowel regimen and constipation prevention, especially on opioids — fluids, fiber, activity, and the prescribed regimen.
  • Ostomy care with return demonstration — appliance changes, stoma and skin monitoring, and output expectations.
  • Recognizing GI bleeding and an acute abdomen and when to call or activate emergency response.
  • Diet and hydration for GI health (with Nutrition and Hydration) and the GI effects of medications.
  • When to report a change in bowel function or new symptoms.
