4. Normal findings

The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.

  • Abdomen soft, non-distended, and non-tender.
  • Bowel sounds present and normoactive in all four quadrants.
  • No masses, guarding, rigidity, or rebound.
  • A regular bowel pattern at the patient's baseline.
  • No nausea or vomiting.
  • Appetite adequate (detailed intake in Nutrition).
  • No signs of GI bleeding.
  • An ostomy, where present, with a healthy stoma, appropriate output, and intact peristomal skin.
