10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

  • The effect of a bowel intervention — a bowel movement after the regimen, or relief of constipation.
  • Ostomy-care competence after teaching — a return demonstration of the appliance change.
  • Teaching effectiveness: teach-back of the bowel regimen and the warning signs.
  • The response to nausea management or diet and hydration coordination.
  • The plan for reassessment timing and what will be re-evaluated next visit.
