Response and reassessment Document the patient’s response in measurable terms — correct return demonstration of drop technique, functional vision compared to baseline, an emergency recognized and routed — not “tolerated well.”
State the reassessment plan: the functional-vision and adherence parameters to recheck, the surveillance schedule, and the threshold for emergency escalation. When a sight-threatening change occurs, document the revised plan and the closed-loop escalation.
