Physician or provider notification Notify the provider for a new gradual visual decline, an external-eye infection, or non-adherence to a glaucoma or post-operative regimen; document the finding, the time, the orders received, and the follow-through.
Escalate emergently — to emergency eye care or EMS — for sudden painless vision loss, acute angle-closure glaucoma, retinal-detachment symptoms, a visual deficit with other neurologic signs, or a chemical exposure, and document the closed loop.
