10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

  • The effect of an intervention — improved sleep after a contributor is addressed, or reduced grogginess after a sleep-aid change.
  • Tolerance and adherence of a CPAP or sleep-aid change.
  • Teaching effectiveness: teach-back of sleep hygiene and sleep-aid safety.
  • The functional and mood response to improved sleep.
  • The plan for reassessment timing and what will be re-evaluated next visit.
