A complete assessment addresses each of the following each visit, scaled to the patient's diagnosis and stability:
Sleep quantity — total hours, sleep latency, the number and cause of awakenings, and early-morning waking.
Sleep quality — whether sleep is restorative, and the degree of daytime sleepiness or fatigue.
The sleep pattern and schedule and the sleep environment.
Contributing factors — pain, nocturnal dyspnea, nocturia, anxiety or depression, restless legs, medications, caffeine, and daytime napping.
Sleep-disordered-breathing signs — snoring, witnessed apneas, gasping, morning headache, and excessive daytime sleepiness (a screen such as STOP-Bang).
Current sleep aids or medications — effect and safety, with particular attention to sedative-hypnotic fall risk in older adults.
The impact of sleep on function, mood, and safety, and CPAP or BiPAP adherence where applicable.
