5. Abnormal findings

Abnormal findings and what they may signal — each warrants Mode B focused documentation and, where parameters are met, intervention and notification:

  • Insufficient or nonrestorative sleep.
  • Insomnia — difficulty with sleep onset, maintenance, or early-morning waking.
  • Excessive daytime sleepiness or fatigue affecting function.
  • Sleep-disordered-breathing signs — snoring, witnessed apneas, gasping, or morning headache.
  • Identifiable contributing factors — pain, nocturnal dyspnea, nocturia, mood, restless legs, medications, or caffeine.
  • An unsafe or ineffective sleep aid, including a sedative-hypnotic creating fall risk.
  • CPAP or BiPAP nonadherence, and a functional, mood, or safety impact of poor sleep.
