Response and reassessment Document the patient’s response and the surveillance outcome in measurable terms — serial neuro check unchanged, precautions verbalized, a lesion measured and trending — not “tolerated well.”
State the reassessment plan: the neuro-check schedule after a strike, the parameters to recheck, and the threshold for emergency escalation. When an evolving sign appears, document the revised plan and the closed-loop escalation.
