- Any head strike or fall, the mechanism, and the time it occurred.
- Headache — new or changed, its severity and quality, and any sudden onset.
- Vomiting, drowsiness, confusion, vision change, or weakness since a strike, and adherence to head-injury precautions.
- In older adults, scalp tenderness, jaw pain with chewing, or visual change.
