11. Physician or provider notification

Notify the provider for a scalp infection, a non-healing lesion or new mass, or a headache that is new or changed but without immediate red-flag features; document the finding, the time, the orders received, and the follow-through.

Escalate emergently and activate EMS for any evolving sign of intracranial injury after a strike (especially on an anticoagulant), a thunderclap headache, giant-cell-arteritis with visual change, or suspected meningitis, and document the closed loop.
