Response and reassessment Document the patient’s response in measurable terms — a correct return demonstration of the foot check, a new lesion identified and offloaded, compensation strategies in use — not “understands neuropathy.”
State the reassessment plan: the sensory parameters and skin areas to recheck next visit, the expected trajectory, and the threshold for escalation or referral. When protective sensation is newly lost or an injury is found, document the revised plan and the coordination that produced it.
