The patterns below are the system-specific failure modes; each maps to Segment 6 (Denial Patterns).

• Sensation charted qualitatively (“okay,” “no numbness”) without testing, the monofilament screen, or comparison to baseline.
• A diabetic or neuropathic foot charted without a documented protective-sensation status.
• A new lesion in an insensate area noted without offloading, notification, or follow-through (no closed loop).
• Repetitive, cloned sensory notes with identical wording and no trajectory.
• Teaching documented as “educated on foot care” without learner response or return demonstration.
• A stable chronic deficit charted without an articulated ongoing skilled need (no injury risk, teaching gap, or management rationale).
