Scenario

Model daily note (edit to the shift)

Worsening dysphagia

New coughing and wet voice with pocketing (or low-grade fevers/respiratory change suggesting silent aspiration) — intake held, positioned/monitored, SLP/provider notified, swallow re-eval + diet modification ordered, precautions reinforced, respiratory watch; reassess; supports Section K.

Aspiration event

Coughing/choking during a meal with color change and new congestion — meal stopped, positioned/suctioned, SpO₂/lungs assessed, provider notified, chest film ordered, precautions intensified, representative notified; reassess; supports Section K. (Respiratory.)

Complete dysphagia

Unable to safely manage the ordered diet or secretions — NPO per order, provider/SLP notified, alternative-nutrition and secretion/airway plan considered, monitoring; supports Section K; skilled escalation.

Linked disease-specific scenarios
