5. Skilled interventions and teaching

Skilled interventions

  • Assess swallowing and aspiration risk at meals (including for silent aspiration) and manage the aspiration precautions and diet texture.
  • Coordinate the SLP swallow evaluation and carry over the plan; manage safe diet-texture progression.
  • Recognize and immediately manage an aspiration event with positioning, suction, oxygen, and escalation.

Teaching / training (to mastery)

  • Teach the resident/representative the aspiration precautions, diet-texture and liquid-consistency requirements, safe-swallow strategies, and warning signs, with return-demonstration/teach-back.
  • Train CNAs on aspiration precautions, safe feeding technique, and reporting coughing/wet voice/pocketing or a respiratory change, with return-demonstration.
