Why it fails

Strong — defensible

“Legs swollen. Compression on. Pulses present.”

No grade, no comparison, no perfusion detail, no skilled rationale.

The venous-insufficiency model note in §3: edema grade vs prior, correct compression and elevation, pulses and skin, and the reason daily vascular assessment was required.

“Compression applied.”

No arterial-adequacy check, no tolerance, no effect.

“Ordered compression applied correctly after confirming palpable pulses; tolerated; edema improved to 1+ from 2+.”

“Calf tender.”

No comparison, no measurement, no DVT workup.

The DVT model note in §4: unilateral swelling/warmth/tenderness with calf-circumference comparison, provider notification, Doppler ordered, embolic monitoring.
