Acute limb ischemia — the six P’s (pain, pallor, pulselessness, paresthesia, poikilothermia/cold, paralysis) — emergency evaluation/transfer; do not apply compression.

Pulmonary embolism — sudden dyspnea, pleuritic pain, tachycardia, hypoxia (with a suspected DVT) — escalate/transfer.

Major bleeding on anticoagulation for a DVT — escalate/transfer; route to the Anticoagulation overlay.

Rapidly worsening ulcer or gangrene — spreading necrosis, dusky/black tissue, or signs of infection — notify the provider urgently.
