7. Skilled-need justification (SNF)

Map which Chapter 8 bases apply, then show the need is daily and can be met only in a SNF (the practical-matter test). This is what a coverage reviewer looks for on a Part A day.

Observation & assessment of an unstable condition

A PAD/limb-ischemia risk, a possible or confirmed DVT, or a progressing vascular ulcer carries a reasonable potential for a time-critical change; assessing perfusion, edema, and DVT signs requires daily nursing judgment.

Covered skilled procedure; management & evaluation

Compression therapy and complex vascular-ulcer care are skilled services under Chapter 8 when required, and managing the vascular plan is skilled.
