Response and reassessment Document the patient’s response and the device outcome in measurable terms — patency restored, dressing intact, output measured and trending, infusion delivered without reaction — not “tolerated well.”
State the reassessment plan: the site, patency, securement, and output parameters to recheck next visit, the expected trajectory, and the threshold for escalation, line change, or removal. When a complication occurs, document the revised plan and the coordination that produced it.
