Every device entry should make the skilled need explicit. The medical-necessity formula combines the skilled action, the RN-level reason it required a nurse, and the measurable effect on the patient or the plan.

Worked examples

A skilled sterile dressing change and patency assessment of a PICC were required to maintain central access for home antibiotics and to confirm the absence of infection and occlusion, allowing therapy to continue safely at home.

Skilled assessment of a PICC site was required to identify new purulence and erythema with fever, prompting cultures and provider notification that led to line removal and antibiotics before the bloodstream infection progressed.

Skilled recognition of sudden dyspnea during line flushing was required to clamp the line, position the patient, and activate EMS for a suspected air embolism, preventing a fatal outcome.
