“Normal/at-baseline” means the device is functioning, the site is uncomplicated, and care is being delivered on schedule. Each line states the finding, the baseline comparison, and the skilled reason a nurse was required — a bare “line intact” does not qualify.
Site clean, dry, and intact without erythema, swelling, tenderness, drainage, or bleeding.
Device patent — flushes without resistance and, for vascular access, draws a brisk blood return where assessed.
Securement intact and the external length or marking unchanged, with no migration or dislodgement.
Dressing clean, dry, intact, and changed on schedule with sterile technique.
Drain output or enteral feeding within the expected range and tolerated, where applicable.
Tubing and connections intact, without kinks, leaks, or disconnections; the closed system maintained.
No local or systemic signs of device-related complication, and the device plan appropriate to continue.
