- Difficulty with specific tasks — bathing, dressing, toileting, transfers, meal preparation, or medication management.
- Falls or near-falls during activities (detail in Safety).
- Caregiver availability, the assistance provided, and any gaps or strain.
- Barriers — pain, fatigue, weakness, cognition, or the home environment — that limit function.
