Subjective — ask

• What the patient can and cannot do independently, and any change since the prior visit.
• Difficulty with specific tasks — bathing, dressing, toileting, transfers, meal preparation, or medication management.
• Falls or near-falls during activities (detail in Safety).
• Caregiver availability, the assistance provided, and any gaps or strain.
• Barriers — pain, fatigue, weakness, cognition, or the home environment — that limit function.
