10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

  • The effect of an intervention — for example improved transfer safety after technique training or a device adjustment.
  • Caregiver competence after training — a return demonstration of safe assistance.
  • Teaching effectiveness: teach-back percentage or return-demonstration result for transfers or device use.
  • Functional response — a change in the assistance level or in task performance.
  • The plan for reassessment timing and what will be re-evaluated next visit.
