5. Abnormal findings

Abnormal findings and what they may signal — each warrants Mode B focused documentation and, where parameters are met, intervention and notification:

  • A new or increased need for assistance with one or more activities of daily living or instrumental activities.
  • Impaired or unsafe transfers, or an inability to bear weight or transfer.
  • Gait instability and fall risk (assessed in Safety).
  • Functional decline from baseline — a loss of a previously independent task.
  • Inappropriate, missing, damaged, or incorrectly used durable medical equipment.
  • An inability to manage a critical instrumental activity — for example medication management or meal preparation — that affects safety.
  • A caregiver gap that leaves required assistance unmet.
