Document the skilled interventions delivered this visit; each should connect to a finding and carry a reassessment in Section 10.

• Functional assessment with grading of the assistance level and interpretation of any change from baseline.
• Transfer and mobility evaluation with instruction in safe techniques.
• Durable-medical-equipment and assistive-device assessment, recommendation, and training.
• Coordination of a physical- or occupational-therapy referral per order when function is impaired or declining.
• Caregiver training in safe assistance and task adaptation, and linking the findings to the plan of care and homebound documentation.
