Normal findings The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.
Activities of daily living performed at the patient's established level of assistance. Instrumental activities managed independently or with the established support. Transfers and ambulation at baseline with the appropriate assistive device. No new functional decline from baseline. Durable medical equipment appropriate, in good condition, and used correctly.
