4. Normal findings

The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.

  • Activities of daily living performed at the patient's established level of assistance.
  • Instrumental activities managed independently or with the established support.
  • Transfers and ambulation at baseline with the appropriate assistive device.
  • No new functional decline from baseline.
  • Durable medical equipment appropriate, in good condition, and used correctly.
  • Caregiver support adequate for the assistance the patient requires.
  • The support plan adequate to maintain safety.
