Response and reassessment Document the patient’s response in measurable terms — correct return demonstration of hearing-aid care, functional hearing compared to baseline, a central vertigo recognized and routed — not “tolerated well.”
State the reassessment plan: the functional-hearing and balance parameters to recheck, the hearing-aid competency to confirm, and the threshold for escalation. When a time-sensitive change occurs, document the revised plan and the closed-loop escalation.
