• New weakness or numbness — location, onset (sudden versus gradual), and progression.
• Speech, swallowing, or vision changes; new or severe headache; dizziness or vertigo.
• Seizure activity — description, duration, and post-ictal state.
• Falls or balance problems (detail in Safety and Function).
• Adherence to and effects of neurologic medications (for example antiepileptics or Parkinson's medications).
