Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.
- The outcome of an emergency activation, or the effect of an intervention on a deficit.
- Seizure frequency and medication tolerance after reinforcement.
- Teaching effectiveness: teach-back of the stroke warning signs and the 911 imperative.
- Whether the neurologic status has changed and what it indicates.
- The plan for reassessment timing and what will be re-evaluated next visit.
