5. Abnormal findings

Abnormal findings and what they may signal — each warrants Mode B focused documentation and, where parameters are met, intervention and notification:

• Altered level of consciousness or mental status (with Cognitive / Mental Status).
• Cranial-nerve deficits — facial droop, dysarthria, dysphagia, a visual-field cut, a pupil abnormality, or an extraocular-movement deficit.
• Motor weakness, asymmetry, pronator drift, or abnormal tone.
• Sensory deficits (detailed mapping in Touch / Sensation).
• Incoordination or ataxia.
• Gait instability or a new inability to walk.
• A focal deficit or a positive stroke screen — a time-critical emergency.
• Seizure activity, a new or worsening severe headache with neurologic signs, or any change from the neurologic baseline.
