The nervous system governs movement, sensation, cognition, and the protective reflexes that keep a patient safe, and a change can be the first sign of a stroke, a seizure, or rising intracranial pressure. The decisive skilled act is recognizing an acute focal deficit — a new facial droop, arm weakness, or speech change — and activating emergency response without delay, because the treatment window for acute stroke is measured in minutes. That recognition and the systematic examination behind it require nursing judgment.
The purpose of this module is to give clinicians the skilled framework and chartable language that demonstrate the neurologic examination, acute-event recognition, and deficit monitoring were reasonable and necessary and required the skills of a licensed nurse. It is the primary home for the neurologic examination; detailed cognition is cross-linked to Cognitive / Mental Status, detailed sensation to Touch / Sensation, and the functional impact to Function and Musculoskeletal.
