10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

  • The effect of resource coordination — a need met, a service arranged, or a barrier reduced.
  • The caregiver's response to support or respite, and any change in strain.
  • Teaching effectiveness: teach-back of the resources and coping strategies.
  • Whether the psychosocial situation is more stable or what remains to be addressed.
  • The plan for reassessment timing and what will be re-evaluated next visit.
