6. Mode A — complete chartable narrative
Use when the system is unremarkable or at baseline. Fill the bracketed blanks with the visit's actual values; the clinical findings are not changed — the reasoning a layperson cannot supply is added. The skilled-rationale portion supports defensibility when it is accurate, individualized, consistent with the plan of care, and tied to the findings, diagnosis, risks, interventions, and patient response documented during the actual visit.
Mode A — structured narrative example · edit to the actual visit Skilled psychosocial and caregiver assessment performed. The support system is adequate for the patient's needs, and the primary caregiver is capable, willing, and competent without significant strain. The patient is coping and adjusting adequately to the illness. The social determinants of health are stable — housing, food security, finances, and transportation — with no unmet needs and no financial barrier to the plan; advance-care-planning has been addressed as appropriate. There is no isolation concern and no indicator of abuse, neglect, or exploitation. RN-level assessment was required to evaluate the support system and the caregiver's capacity and strain, assess coping, screen the social determinants of health, and coordinate the resources the plan of care depends on, given the diagnosis of [______]. Skilled nursing reinforced caregiver support and coping resources and the changes requiring a call, and will continue monitoring the psychosocial situation. Continued skilled observation and assessment indicated. |

Select only the elements actually assessed and applicable; mood and risk are documented in Psychiatric / Mental Health, functional dependence in Function, and any abuse reporting in Safety. The social-determinant screen and the caregiver-strain assessment are charted as performed and relevant to the diagnosis and the plan.
