A clinically perfect plan fails if the patient has no one to help, a caregiver who is breaking down, no transportation to the pharmacy, or no money for food and medications. Social support, caregiver capacity, coping, and the social determinants of health are decisive for safety, adherence, and outcomes, and assessing them and mobilizing resources is skilled nursing work — often the management and evaluation of the plan that holds the whole episode together.
The purpose of this module is to give clinicians the skilled framework and chartable language that demonstrate psychosocial assessment, caregiver-capacity and strain evaluation, and resource coordination were reasonable and necessary and required the skills of a licensed nurse. It is the primary home for psychosocial assessment; mood and risk are cross-linked to Psychiatric / Mental Health, functional dependence to Function, and abuse reporting to Safety.
