5. Abnormal findings

Abnormal findings and what they may signal — each warrants Mode B focused documentation and, where parameters are met, intervention and notification:

• An inadequate or absent support system.
• Caregiver incapacity, unwillingness, or significant strain or burnout.
• Poor patient coping or difficulty adjusting to the illness.
• Social-determinant barriers — housing instability, food insecurity, financial hardship, or no transportation.
• Unmet needs requiring resources, or a financial barrier to medications or care.
• Social isolation or loneliness.
• Indicators of abuse, neglect, or exploitation (mandated reporting is handled in Safety).
