Abnormal findings and what they may signal — each warrants Mode B focused documentation and, where parameters are met, intervention and notification:
- An inadequate or absent support system.
- Caregiver incapacity, unwillingness, or significant strain or burnout.
- Poor patient coping or difficulty adjusting to the illness.
- Social-determinant barriers — housing instability, food insecurity, financial hardship, or no transportation.
- Unmet needs requiring resources, or a financial barrier to medications or care.
- Social isolation or loneliness.
- Indicators of abuse, neglect, or exploitation (mandated reporting is handled in Safety).
