4. Normal findings

The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.

  • An adequate support system for the patient's needs.
  • A capable, willing caregiver without significant strain.
  • The patient coping and adjusting adequately to the illness.
  • Stable housing, food security, finances, and transportation.
  • Adequate resources with no unmet needs and no financial barrier to the plan.
  • Advance-care-planning addressed as appropriate to the situation.
  • No isolation concern and no indicator of abuse, neglect, or exploitation.
