11. Physician or provider notification

Notify the provider per the patient-specific parameters and agency policy, and document the closed loop — contact, content, response, and follow-through.

  • State the specific parameter that was met (the value or finding that triggered notification).
  • Record the date and time of contact, who was notified, exactly what was reported, the response or orders received, and the follow-through.
  • If a response is pending, document the scheduled follow-up time so the loop is not left open.

Common notification triggers for this system: a caregiver breakdown that leaves the patient unsafe; a social crisis threatening safety (housing loss, no food or utilities, or no medication access); a patient in psychological crisis (coordinated through Psychiatric / Mental Health); or any indicator of abuse, neglect, or exploitation (mandated reporting handled in Safety).

Closed-loop example

“Provider notified at 12:30 that the primary caregiver is experiencing significant strain and can no longer safely meet the patient's care needs, leaving a gap. Order received to coordinate additional personal-care support and respite and to evaluate the level of care. Social work was engaged, interim supports were arranged, and caregiver respite resources were provided; reassessment arranged.”
