• Cleansing and the skilled dressing change per order, using clean or sterile no-touch technique.
• Sharp or conservative debridement only within scope and order; otherwise notify for a debridement order.
• Offloading and pressure-redistribution measures, or compression for venous disease per order.
• Wound measurement and trajectory tracking, with photography per policy.
• Pain management before and during care per order.
• Coordination with the provider, wound-care specialist, or therapy when the plan needs revision.
