Physician or provider notification Notify the provider for deterioration, new infection signs, exposed bone, a new unstageable injury or DTI, uncontrolled pain, or a wound that is not progressing on the current order; document the finding reported, the time, the orders received, and the follow-through.
Escalate emergently and activate EMS for dehiscence with evisceration, signs of a necrotizing infection, or systemic sepsis, and document the closed loop.
