Subjective — ask

• Pain at the wound at rest and with dressing changes, and any change since the last visit.
• New drainage, odor, bleeding, or dressing failure between visits.
• Adherence to offloading, positioning, compression, nutrition, and activity instructions.
• Any fever, chills, or feeling unwell.
