Wound care frequently supports both homebound criteria; document the patient-specific evidence when it is present and do not overstate it.
Wound-related evidence that supports homebound status
- Extensive wound-care needs and complex dressing regimens that the patient cannot self-manage.
- Offloading or non-weight-bearing requirements that make leaving home unsafe or require considerable, taxing effort and assistance.
- Pain, exudate, or device burden (e.g. negative-pressure therapy) that limits the ability to leave home.
- A need for assistance and a taxing effort to access wound care outside the home.
