Response and reassessment Document the patient’s response to the dressing change and pain management in measurable terms — pain reduced from one score to another, dressing tolerated, no bleeding — not “tolerated well.”
State the reassessment plan: the parameters to re-measure next visit, the expected trajectory, and the threshold at which the plan changes. When healing has stalled or reversed, document the revised plan and the coordination that produced it.
