Wounds and pressure injuries are among the most common reasons patients receive skilled home health, and they are a frequent target of medical-review denials because the documentation often records what the wound looked like without proving why a nurse was required to assess and treat it. A wound that is measured, staged, compared to baseline, and tied to a treatment order is evidence of skilled care; a wound that is merely described is not.
The purpose of this module is to give the clinician the skilled framework — etiology classification, NPIAP staging, objective measurement, wound-bed and periwound assessment, infection surveillance, and the closed-loop response to change — and the chartable language that demonstrates observation and assessment, skilled procedure, teaching, and management and evaluation of the plan of care on every visit.
